Procedure:
Please read through all steps:
1. Check Coverage Page for current fund availability (There is no due date for the application, but funds will run out)
2. Fill the Application out in Word or print and fill out.
3. Email or mail application.
4. Wait two to four weeks for confirmation of award (this time frame will be shorter if form is emailed)
5. Make purchase/pay expense.
6. Fill out reimbursement form and include receipts (If purchase was already made within the last month before the application is filed, receipts may be attached to the original application).
7. Wait two to four weeks for reimbursement check.
Note: All applications are confidential and will not be shared with anyone outside of the review committee, composed of the current Fund President and Vice President. If you wish to make a public statement with regard to the importance of this program, simply email it to us and include your full name in the email.
